Childhood Immunisations
It is very important that you keep the appointment for your Child's Immunisations. It protects them and every other child they have contact with.

New Patient Registration

registration4To register with us you will need to complete a couple of forms that are available from our administration team. As we take the privacy of your health records very seriously we do need to confirm your identity. When you wish to register we will ask to see documentation to confirm this. We will need to see something official such as a rent book, council tax demand, bank statement or utility bill that must have your current address on it. Where possible we would like to see official Photo ID such as a passport, new style driving licence or student matriculation card. When these cannot be provided we will ask for further documentation.

We will then make you an appointment to see one of our nurses for a new patient health check. You are not considered to be registered at the practice until you have had this health check.

Children under the age of 13 are normally exempt from this health check.
